Welcome to Thompson River Pediatrics! We are excited to get to care for your children like we do our own. Please print off and fill out the first 4 forms and bring them to your first appointment. If you have records that you would like to transfer from another providers office you can also fill out the 5th form and bring it with you and we will fax it to your previous provider for you to receive your child's records.

We are in network with all of the Major plans, however it is always best to call your specific insurance provider to verify that Thompson River Pediatrics is in Network before calling our office to schedule.
